

Helping young people transition from school
Leaving school is a big milestone and it often comes with changes to how your child’s NDIS supports are managed. Here, we outline what you need to know and do to ensure a smooth transition.
When your child turns 18
- Under the NDIS, your young person is now considered an adult.
- This means they’re expected to manage their own plan unless a Plan Nominee or formal support is in place.
- Many families aren’t aware of this shift until services are delayed or paused – so early action is key.
What to do - and when
We recommend acting by June in your child’s final year of school, or at least 6 months before they turn 18.
This gives you time to:
- Request a Change in Circumstances with the NDIS. Visit Changing your plan.
- Set up any formal roles (like a Plan Nominee) if your child needs help managing their plan.
- Start gathering evidence for individual support needs after school to discuss with NDIS – Local Area Coordinator.
- Begin exploring options like day programs, SLES (School Leaver Employment Supports), life skills and community programs.
Simple steps to get started
1. Review the current plan
Is it up to date and does it include adult goals, like building independence, employment or community involvement?
2. Contact the NDIS
Call 1800 800 110 or speak with your Local Area Coordinator (LAC) to request a Change in Circumstances and update the plan.
3. Set up a Plan Nominee (if needed)
If your child needs help managing their NDIS plan after 18, you can apply to become their Plan Nominee. Visit the NDIS Guidelines website to learn more.
4. Consider individual supports of interest
Think about the next steps of what your child may be interested in exploring:
- Day programs
- Employment supports – School Leavers Employment Supports
- Life skills and community engagement
These may need specific funding in the plan.
